Perimenopause palpitations: what your watch can show

You’re sitting still, maybe just drifting off, and suddenly your heart is pounding, fluttering or skipping. One Watch sample cannot explain an episode. But repeated timing can give you something much better than a vague memory: symptom dates, resting-heart-rate readings, sleep context and what happened on comparison days. What palpitations usually are

A palpitation is an awareness of your own heartbeat — fast, hard, irregular, fluttering or skipped. Palpitations are reported during the menopause transition, and studies describe group-level cardiovascular and autonomic changes in midlife. Those findings provide context, but the most useful personal lead comes from your own repeated timing: when episodes happen, what else was going on and whether the same relationship returns. What your Apple Watch can actually tell you

Your watch checks your heart rate periodically in the background, with timing that varies by activity, and uses those readings to calculate a daily resting heart rate. During a workout its optical sensor measures continuously; an ordinary night is not a continuous recording. It can flag unusually high or low rates, or capture an ECG on supported models if you start one during an episode, but a brief palpitation may fall between background samples.

That does not make the background record useless. Across several symptom dates, you can compare resting heart rate, sleep and available samples with other days. First, know when not to wait

Palpitations with chest pain, shortness of breath, fainting or near-fainting, or an episode that will not settle require prompt medical attention, not interpretation in an app. New, persistent, worsening or concerning palpitations also deserve clinical evaluation.

When none of those urgent signs is present, a dated log can make the next conversation much more useful. Note when the episode started, roughly how long it lasted and the context that matters — waking from sleep, recent caffeine, illness or another symptom. The goal is not to diagnose yourself. It is to see which relationship repeats often enough to deserve the first question.

How Perigee reads it

One gentle thing

You don’t need to overhaul anything today. If palpitations show up often, jot down when they happen — after coffee, in the small hours, in the days before your period — and let your Watch keep the heart-rate record alongside. Are heart palpitations normal in perimenopause?

Palpitations are reported during the menopause transition, but one episode does not identify its cause or safety. Can my Apple Watch capture a palpitation?

When should I see a doctor about palpitations?

Chest pain, fainting or near-fainting, breathlessness, or a racing heart that will not settle are reasons to seek medical care promptly — not to open an app. If palpitations are frequent but not frightening, bringing your watch’s heart-rate history to a routine appointment gives the conversation something concrete.

Does a higher resting heart rate mean something is wrong?

Not by itself. One higher daily estimate is a data point. If higher resting-heart-rate days repeatedly overlap with logged palpitations while comparison days do not, that relationship becomes a stronger lead to review.
